Deputies observed Chapman’s eyes appeared droopy and his speech slurred. A series of field sobriety tests were conducted, and based on Chapman’s performance, he was arrested. Chapman was booked into the John E. Polk Correctional Facility on charges of Child Neglect, Driving Under the Influence, and Leaving the Scene of a Crash. He remains incarcerated on a $6,000 bond. No injuries were reported as a result of the accident.
Arrestees: Alexander Lewis Chapman (DOB 10/08/88), Creekview Circle, Oviedo.

REPORT FROM ARRESTING DEPUTY ON STATEMENT OF CHARGES:
On March 26, 2017, at approximately 2112 hours, I responded to a vehicle crash located at 4661 Creekview Lane. The crash was reported as a vehicle versus a tree. Upon arrival, I observed gold Hyundai bearing the tag: GGEJ53 that had been crashed into a tree. I then conducted the crash investigation and located the driver of the vehicle, which was located at 2537 Creekview Circle, which is not his residence. The driver, Alexander Lewis Chapman (D.O.B 10-08-1988), identified by his Florida Driver’s License, stated he was taking out for a ride. Alexander advised his mirror was hanging from his front windshield and caused him to misjudge the turn causing him to hit the tree. After the crash investigation was complete, I informed Alexander the investigation for the crash was complete and notified him that I am now starting an investigation for DUI. I then asked Alexander, the driver of the gold Hyundai, to walk over to the front of my patrol vehicle and he complied. Once at my patrol vehicle Alexander was read his Miranda warnings off my department issued card. Alexander stated that he understood each of his rights and willingly agreed to talk to me. During conversation with Alexander, I observed that he had slurred speech, and droopy eyes.

I then asked if Alexander, if he was willing to perform field sobriety exercises. Alexander stated that he was willing to. Each exercise was both demonstrated and explained before the defendant was asked to perform the exercise. Alexander verbally replied that he understood each exercise before he started. Before the exercises started, I asked Alexander, do you have any physical limitations that would hinder your performance on the field sobriety exercises? He stated that he has glasses but that he was currently wearing contacts. He also advised that he would do the exercises barefoot, due to it being more comfortable. I asked the defendant, do you have any problems with your arms, legs, or balance? The defendant stated, No. The field sobriety exercises were done on a flat and level surface, free from debris, and my emergency lights and siren turned off. The first exercise was the (HGN) Horizontal Gaze Nystagmus. During the exercise I observed the following: (No glasses) Pupils were equal in size Checked for equal tracking, both eyes tracked equally -Lack of smooth pursuit in both the Left and Right eyes -Onset Nystagmus prior to 45 degrees in both eyes -Checked Vertical Gaze Nystagmus -Eyes were glassy, watery. -Swayed while standing still -Unable to follow instructions: did not maintain contact with tip of pen. The second exercise was the (WAT) Walk and Turn. During the exercise I observed the following: -failed to count steps -Failed to stay on line while walking -failed to look down while he walked -stopped during exercise to verify if he was doing it correct -Can`t keep balance -Missed several heel to toe steps -Stepped off the line -Used arms to balance, raised arms over six inches The third exercise was the (OLS) One Leg Stand. (Used his right leg for the exercise) During the exercise I observed the following: -Swayed while balancing -Used arms to balance, raised arms more than six inches -Unable to follow instructions. Dropped foot multiple times -Even after the defendant put he foot down, I informed he to do the above listed instructions and he still failed to perform the divided attention tasks -Placed arms behind back -counted (16) twice The fourth exercise was the Finger to Nose (FTN). (R,L,L,R,L) During the exercise I observed the following: -Failed to return arms to the side (on the first attempt) -movements were very slow -Failed to return arms to the side -Tip of Index finger did not touch tip of the nose on ALL attempts -Touched the left side of nose with right hand The fifth exercise was the Rhomberg Alphabet: (B through X) During the exercise I observed the following: -orbital sway -Skipped letters between R-X.

After the exercises were completed, I determined the defendant`s normal faculties required to operate a motor vehicle were impaired beyond the legal limit by either alcohol and or drugs. Therefore, the defendant was arrested for Driving Under the Influence of Alcohol and or Drugs, F.S.S. 316.193. Due to Alexander not having in a proper child restraint and facing the correct way, he did not provide with the care, supervision, and services necessary to maintain the child’s physical and mental health. Alexander also was seen leaving in the car at the scene of the traffic accident.
Due to the facts and circumstances, Alexander is in violation of F.S.S 827.03 (2d) – Neglect child without great bodily harm.
Alexander was seen by an off-duty fireman, leaving the scene that had more than $50 damage, due to the airbag deployment and the front end damage, which is in violation of 316.061 (1)- Hit and run- leave the scene of a crash involving damage to property greater than $50. Alexander was placed in handcuffs that were checked for proper fit and double locked.
The defendant was transported to the John E. Polk Correctional Facility Upon arrival at the jail, the defendant was turned over to the Breath Technician, Ray Garcia.
The defendant was asked to provide a lawful sample of his breath and urine and he refused. Mr. Garcia read the defendant Implied Consent, the defendant acknowledged the consequences, and still refused to provide a breath sample and urine sample.
Due to him being in a car accident he needed to medically cleared. Alexander was taken to Central Florida Hospital, cleared, and returned to the John E. Polk Correctional Facility.

Trevor J. Olivier, 31, was traveling along Weeks Island Road around 5 a.m. when he crashed the vehicle into a residence in the 8000 block.

Daniel Charles of Lake Charles Arrested for Fourth Offense DWI

VINTON, LA—Troopers arrested a Lake Charles man for fourth offense DWI this morning after he drove a vehicle into a ditch.

Darrel Carter of Lake Charles 4th DWI offense Louisiana State Police crashed into ditch 030717

On March 7, 2017, shortly after 5:00 a.m., Louisiana State Police Troop D responded to a report of a vehicle in a ditch near Louisiana Highway 109 just south of I-10 west of Vinton. A Trooper determined the driver, 45-year-old Darrel Carter of Lake Charles, drove into the ditch and was impaired. Carter later refused to provide a toxicology sample. The Trooper requested and obtained a “no refusal” warrant from a judge and subsequently obtained a toxicology sample from Carter.

Carter, who has been previously arrested on numerous occasions for DWI, was charged with careless operation of a motor vehicle and DWI fourth offense (the maximum he can be charged with under Louisiana law). Carter was booked into the Calcasieu Correctional Center.

The Louisiana State Police has a process in place, in cooperation with prosecutors and judges, to request a search warrant in conjunction with a DWI arrest. If an arrested individual refuses to provide a toxicology sample, the circumstances of the arrest may be forwarded to a judge for review. If the judge determines probable cause exists, a “no refusal” warrant is issued by the judge. A qualified medical professional then obtains a toxicology sample without the consent of the arrested individual.

In 2015, 64 percent of motorists who died in crashes investigated by Troop D were killed by an impaired driver.

In 2015, 64 percent of motorists who died in crashes investigated by Troop D were killed by an impaired driver. To report impaired drivers, motorists are encouraged to dial *LSP (*577) from their cellular phone to reach the nearest Louisiana State Police troop location or to contact local law enforcement.

Impaired Driver Fails to “Move Over” and Kills Tow Truck Driver

St. John the Baptist Parish – According to Louisiana State Police, on March 2, 2017, shortly before 11:00 am, Louisiana State Police Troop B investigated a fatal crash on I-55 north at milepost 11.8. The crash involved four vehicles and claimed the life of a tow truck driver, identified as 34-year-old Macy Lee Alvey III of New Orleans, LA.

The Troopers’ initial investigation indicates that a 2003 Chevrolet Silverado, occupied by 53-year-old Kevin Brown of New Orleans, LA, was disabled on the shoulder of I-55 north. A tow truck, 1991 Ford F-450 owned by Rony’s Towing and Recovery LLC, was parked in front of the Chevrolet Silverado rendering assistance. A 2012 Nissan Rogue, driven by 48-year-old Kimela Hall of Norco, LA, was traveling in the right lane of I-55 north and sideswiped the Chevrolet and tow truck. Alvey was standing on the driver’s side of the tow truck and was impacted by the Nissan. The Nissan Rogue then impacted the right guardrail and overturned once, coming to rest across both lanes of I-55 north. The Nissan Rogue was then impacted by a 2008 Pontiac Grand Am, driven by 59-year-old Nina Kincaid of Hammond, LA, in the left lane.

Macy Lee Alvey III was pronounced dead on the scene. Nina Kincaid and Kimela Hall were transported to North Oaks Hospital. Kincaid suffered critical injuries and Hall suffered moderate injuries. Kevin Brown was not injured.

Impairment is suspected on the part of Kimela Hall. A blood sample was obtained and a toxicology test will be performed by the Louisiana State Police Crime Lab. Hall has been arrested for Vehicular Homicide, 1st Degree Vehicular Negligent Injuring, Failure to Move Over for an Emergency Vehicle, Careless Operation and Suspended Driver’s License. Once Hall is released from the hospital, she will be booked into the St. John the Baptist Parish Jail.

Louisiana’s “Move Over” law requires that drivers, upon approaching any vehicle stopped on or near the highway utilizing warning lights, shall yield the right-of-way by making a lane change into a lane not adjacent to the parked vehicle, if possible with due regard to safety and traffic conditions. If a lane change is not possible, the driver shall slow to a reasonably safe speed before passing the stopped vehicle.

Troopers urge citizens to remember that Louisiana’s “Move Over” law keeps EVERYONE safe. The “Move Over” requirement applies not only to first responders such as law enforcement, ambulance, and fire/rescue crews, but also to highway workers, tow and recovery truck operators, and any vehicle utilizing hazard/warning lights. Please MOVE OVER to keep them safe!

Chattanooga Police arrested Chattanooga Firefighter Jerry Lee Jr. for DUI after crash; 2nd offense after prior DUI provided that he apologize to his employer

CHATTANOOGA, TENNESSEE – When a firefighter rushes to a crash scene, he or she is usually in a fire vehicle or an ambulance, for the purpose of aiding those who are in jeopardy of losing their life.  In the case of Chattanooga firefighter Jerry Lee Jr., police report that he wasn’t ready to help but instead was ready to kill others as he allegedly drove while impaired.

From Times Free Press

April 29, 2017 — A Chattanooga firefighter was arrested Saturday morning after allegedly caused a three-car collision while driving drunk.

Police responded to a report of a car crash near 1400 Cowart Street at 2:55 a.m. and found a Dodge truck had struck a Honda Civic and a Maserati, according to court records.

Several patrons of Coyote Jacks Saloon were nearby and the officer was told by witnesses that the truck tried to leave the scene prior to police arrival and several had to jump out of the way to avoid being hit.

Officers were flagged down and stopped the truck before the driver, Jerry Lee Jr., 44, could leave. He was asked to exit his vehicle and officers noted he had “an unsteady gait, water, red eyes, and a strong scent of an intoxicant coming from his person and breath.”

He was also charged with leaving the scene of an accident, implied consent law and for not having proof of insurance. His bond has been set at $3550 and he has been placed on administrative leave by the Chattanooga Fire Department.

Terrebonne Parish, LA – One man is dead and another has been charged with vehicular homicide after a vehicle struck a pedestrian in Terrebonne Parish last night.

On Monday, April 17, 2017, shortly before 9 p.m., Louisiana State Police Troop C responded to a crash involving a vehicle and a pedestrian on Louisiana Highway 24 near Clendenning Road. The preliminary investigation revealed a 2007 Dodge Caliber driven by 62-year-old Albert Williams Jr. of Montegut was traveling south on LA 24 and drove onto the southbound shoulder, striking a pedestrian. The pedestrian, 43-year-old Byron Acosta of Houma, was struck by the vehicle and suffered fatal injuries. The Terrebonne Parish Coroner’s Office arrived on scene and pronounced Acosta deceased.

Williams was wearing a seat belt and was not injured. Williams was charged with vehicular homicide as well as improper lane usage and was booked into the Terrebonne Parish Correctional Center. The crash remains under investigation.

Wrong-Way Driver Carl Lee Bryant Killed, Impairment Considered a Factor

Calcasieu Parish – Louisiana State Police report that on April 21, 2017, a crash involving two vehicles killed a man from Lake Charles, LA. He was not wearing a seat belt, is suspected of being impaired, and was operating his vehicle the wrong way on the road.

Troopers responded to the crash around 10:10 p.m., which occurred on US Hwy 171 at Topsy Cutoff Road. The crash involved a 1992 Toyota Tercel, driven by Carl Lee Bryant (B/M 61 yrs) and a 2007 Honda CRV, driven by Jennifer M. Fontenot (W/F 27 yrs) of Longville, LA. The Toyota was southbound in the northbound inside lane (left lane) of US Hwy 171 when it collided head-on with the northbound Honda. The Honda was also being driven in the inside lane, but in the correct direction, when the crash occurred.

Bryant was pronounced dead at the scene.

Fontenot was wearing her seatbelt and received moderate injuries. She was transported to Lake Charles Memorial Hospital for treatment.

Routine toxicology tests are pending. The crash remains under investigation.

Motorists must be reminded that alcohol and other drugs have many effects on the body. Alcohol and other drugs can impair visual ability, alter the sense of time and space, impair fine motor skills needed to operate a motor vehicle, and decrease reaction times. Just one drink may cause these effects.

Louisiana State Police would like to take this opportunity to remind/inform motorists that properly wearing your seatbelt will dramatically reduce your chance of being injured or killed in a crash. We ask you to take one second to “buckle up!” Louisiana law requires that every person in a vehicle, regardless of seating position, always remain buckled up. Not wearing a seat belt remains the leading cause of death in motor vehicle crashes.

Troop D has investigated 8 fatal crashes resulting in 11 deaths in 2017.
